Terms of Service Agreement
This Terms of Service Agreement ("Agreement") is entered into between Keen on Nutrition and Rebecca Ourique, a registered dietitian, and the users ("Users" or "You") of the services provided on the website keenonnutrition.com ("Website").
1. Acceptance of Terms:
By accessing and using the services on this Website, you agree to comply with and be bound by the terms and conditions outlined in this Agreement. If you do not agree with any part of these terms, please refrain from using our services.
2. Services Offered:
Keen on Nutrition offers one-on-one counselling, diet analysis, group presentations, and online content to assist individuals in achieving their health and nutrition goals using only evidence-based information and practices.
3. User Responsibilities:
Users are responsible for providing accurate and complete information during consultations, assessments, or course enrollment. You agree to adhere to any guidelines provided by Keen on Nutrition for the optimal implementation of the services.
4. Confidentiality:
All personal information shared during consultations, assessments, or course participation will be treated with strict confidentiality. Refer to our Privacy Policy for details on how your information is handled.
5. Payment and Refund Policy:
Clients are required to pay in full upon booking single sessions to secure their appointment. We understand that unforeseen circumstances may arise, and cancellations are accepted up to 24 hours before the appointment time. However, any cancellations or no-shows outside of this timeframe will incur a 75% charge of the session fee. This policy allows us to maintain the integrity of our services and honor the commitment of both parties.
For clients purchasing a package of services, an initial payment of 50% must be made upon booking. The remaining 50% must be paid prior to our first appointment or an alternative, agreed-upon payment plan can be implemented.
Registered Dietitians are licensed under the College of Dietitians of Ontario. Nutrition services are not covered by provincial health plans at this time (i.e. not yet “OHIP covered”). However, dietitian services may be partially or fully covered by your extended health insurance plan.
The coverage for each insurance company is different; please check your plan to see what coverage is available for this type of service (i.e. dietitian coverage per session or per year). We do not direct bill to insurance companies but will provide you with a detailed receipt you can submit that includes:
- our Registered Dietitian license number
- detailed description of service
- date of the nutrition counselling session
- the consulting fee breakdown per visit
Individual insurance receipts can be provided at each session to break down the nutrition package cost appointment-by-appointment for reimbursement purposes with your insurance provider. If your plan does not cover the services of a Registered Dietitian, save your receipt. In the province of Ontario, Registered Dietitians are classified as Authorized Medical Practitioners and nutrition counselling paid for out-of-pocket counts towards your total medical expenses. Based on your income level, you may qualify for a non-refundable tax credit from the government when you file your taxes.
6. User Conduct:
Users agree to conduct themselves in a respectful and lawful manner during interactions with Keen on Nutrition. Any form of harassment, discrimination, or inappropriate behavior will not be tolerated and services will be terminated without refund.
7. Intellectual Property:
All content, including but not limited to text, images, and materials provided during counselling or online, is the intellectual property of Keen on Nutrition. Users may not reproduce, distribute, or use this content without explicit permission.
8. Termination of Services:
Keen on Nutrition reserves the right to terminate services for any user who violates the terms of this Agreement or engages in behavior deemed unacceptable.
9. Limitation of Liability:
Keen on Nutrition is not liable for any damages, direct or indirect, arising from the use or inability to use the services provided on the Website.
10. Governing Law:
This Agreement is governed by and construed in accordance with the laws of Ontario, Canada.
11. Changes to Terms:
Keen on Nutrition reserves the right to modify or revise these terms at any time. Users will be notified of significant changes, and continued use of the services constitutes acceptance of the updated terms.
By using our services, you acknowledge that you have read, understood, and agree to be bound by these terms.
